Innovative and co‑creative information management for follow‑up of Home for Care or Housing (HVB)
The purpose of the project is to map the information needs in the follow-up of Home for care or housing (HVB) for children and young people, and together with involved actors work out a proposal for a model that can lead to a more efficient information supply.
Providing information means that different target groups must have access to the information they need. It also includes the requirements for handling, organization, information security, storage, retrieval and dissemination of information required for this to be possible.
The starting point is to identify what is needed to be able to create a process-oriented approach that enables follow-up in order to learn and improve the function. It is these information needs that the project will try to identify.
The project is a collaborative project between Forum for Digitization / Mid Sweden University and the municipalities of Örnsköldsvik and Härnösand.
