Feeling safe – homecoming from hospital care to homecare
The purpose of the research of team collaboration during homecoming from hospital care to homecare is to evaluate the person and their relatives' experience of safety, continuity and accessibility, participation, confidence in self-being, health and activity during health and social care.
The home is the own accommodation in in the municipality. As a human being, it is important to be able to be independent, safe, and feel belonging in a social context. If these needs will be meet, ill-health can be prevented when health and social care. are given in the individual home.
The project is to conduct health and social care during team collaboration and at the same time increase the person´s experience of security, through new team collaboration. Health and social care in the project are organized by home care, municipal health care, and/or regional health care through a health centre.
In this projekt, the research project focuses on caring and nursing where the person and their relatives' experience of safety, continuity and accessibility, participation, confidence in self-being, health and activity, rehospitalisation is evaluated.
Qualitative and quantitative data are collected during homecoming and after 6 months.
The project plans to deliver the basis for interim report, final report, workshop to clients of research projects and scientific publications.
